Success of Device-Based Methods for Atrial Tachyarrhythmia Induction during Implantation of an Implantable Cardioverter Defibrillator with Atrial Therapy

چکیده
In addition to standard ventricular therapy, cardioversion using an implantable cardioverter defibrillator (ICD) with atrial therapy is one of the many new options available for treating atrial tachyarrhythmias. New data suggest that shock efficacy for AF depends on identifying atrial defibrillation thresholds (DFT) and programming the first atrial shock to at least two times the atrial DFT [1]. Therefore, to ensure the highest degree of atrial therapy effectiveness, atrial induction testing should be conducted during the initial implantation procedure. In order to successfully induce atrial tachyarrhytmias during implantation or followup, the ICD should have several methods of atrial tachyarrhythmia induction. Atrial arrhythmias contribute to patient morbidity and mortality by impairing cardiac function, increasing the risk of thromboembolic events, and inducing ventricular arrhythmia [2-4]. Implantable cardioverter-defibrillator in a patient with dextrocardia situs inversus
Background: Dextrocardia is a congenital anomaly, which may have coexistent coronary artery disease (CAD), arrhythmias and conventional indications for device therapy. However, the implantation of transvenous leads can be technically challenging and the approach needs to be tailored to the patient's individual anatomy. Case presentation: A 54-year-old male with dextrocardia situs inversus and i...
متن کاملAtrial therapies reduce atrial arrhythmia burden in defibrillator patients.
BACKGROUND Approximately 25% of patients who receive an implantable cardioverter-defibrillator (ICD) to treat ventricular tachyarrhythmias have documented atrial tachyarrhythmias before implantation. This study assessed the ability of device-based prevention and termination therapies to reduce the burden of spontaneous atrial tachyarrhythmias. METHODS AND RESULTS Patients with a standard indi...
متن کاملComparison of a Novel, Single-Lead Atrial Sensing System With a Dual-Chamber Implantable Cardioverter- Defibrillator System in Patients Without Antibradycardia Pacing Indications
Background—Supraventricular tachyarrhythmias are the main cause for inappropriate therapy by implantable cardioverterdefibrillators (ICDs). For better rhythm discrimination, an atrial electrogram is helpful and usually obtained from an additional atrial lead, even in the absence of sinus node or atrioventricular nodal disease. An A -ICD system with integrated atrial sensing rings mounted 15 to ...

BACKGROUND Management of antithrombotic agents after implantable cardioverter defibrillator implantation is challenging, particularly among patients with atrial fibrillation and coronary artery disease. METHODS AND RESULTS Using data from National Cardiovascular Data Registry(®) Implantable Cardioverter Defibrillator Registry(™) linked with Medicare claims data, we identified 25 180 patients ...
متن کاملTrouble Shooting of ICD Therapy: Inappropriate Shock
Patients with an implantable cardioverter defibrillator (ICD) can undergo inappropriate therapies (shock or antitachycardia pacing) if electrical activity not originating in the ventricle is wrongly recognized as ventricular by the device. Inappropriate therapy leading to shocks is not innocuous; consequences include pain and its psychological sequelae, particularly anxiety, as well as inductio...
